The ideal candidate will have a BSW or equivalent with experience in Long Term Care. This position will assist our Social Worker and MDS Co-Ordinator with completion of MDS documentation, psycho-social assessments, care plan coordination, Medicaid re-determinations and other facility projects as needed.
